Now that you have booked your appointment, you need to complete your pre-assessment questionnaire which will provide our clinicians with some information about your health. Using this information, we can provide you with a picture of how healthy you are now, and guidance on how to improve your health for the future.

 

Complete your pre-assessment questionnaire

In just four simple steps your questionnaire can be completed and sent directly to your Bupa Centre.

  1. Please register on our health assessment customer website.
  2. Complete the questionnaire relevant to your health assessment you have chosen.
  3. Submit and view your form. You can print a copy for your own records.

If you are unable to complete the questionnaire online, you can download your questionnaire from the list below to print and complete in your own time.

You can also choose to complete your questionnaire on the day before your assessment. Please arrive at the centre at least 15 minutes before your appointment to allow time to complete this.
