How do I make a claim?

There are three easy steps you will need to follow:

aquire a claim form to take to your dentist. This can be obtained by ringing the BUPA Dental Plan helpline on 0800 237 777* or you can download a copy from the link below
ask your dentist to complete the treatment section on the claim form
return the completed form with original dated receipts to:
