Thank you for your interest in joining the Bupa Approved Physiotherapy Network and agreeing a contract with us.

Providers who are part of the Bupa Approved Physiotherapy Network deliver out-patient, non-hospital physiotherapy services to Bupa insured members. 

This website provides you with all the information you need to apply to join the network.

Step 3) Read the terms and conditions Hide

Please review the terms and conditions (pdf, 55kb) for providing physiotherapy services to Bupa members. You will need to confirm your acceptance of these terms and conditions in order to progress your contract. The future provision of your physiotherapy services to Bupa members will be governed by these terms and conditions.

Step 4) Read the physiotherapy new application - PDF version Hide

We recommend that you read and print this PDF version of the contract..
Please do not return this copy to Bupa.

Step 5) Complete the online application form (opens in a new window) Hide

Once you have confirmed your agreement to the terms and conditions of the contract, a document containing the details you have provided and our standard terms and conditions will be created for you to review and print.

It is your responsibility to ensure that all details are correct and completed online.

Step 6) Submit your application Hide

This will enable us to receive your application electronically. Please complete your application by pressing the ‘Submit’ button. You will be sent an automatic email confirming that we have received your application.

If we do not receive an electronic submission of your application, we will be unable to progress your application.

If your application to join the network is successful, we will send you a contract containing the information you submitted and forward it to you to sign and return.
