How the service works
Our aim is to give you the highest quality of service and support at home, removing as much stress and worry from you and your family, relatives or friends as possible.
Once you have been referred to us for home healthcare by your hospital or GP we will make sure that everything is in place for you to go home and receive the medication and support you need. We work closely with your referring unit and the primary care trust to prepare a package of care that is specifically tailored to meet your needs to make the transition between hospital and home as smooth as possible.
You will be assigned a dedicated patient services co-ordinator who will arrange your initial and ongoing deliveries of all necessary equipment and medication to your home. Your patient services co-ordinator will be available for ongoing support as and when you need it.
All necessary equipment will be delivered to your home prior to you being discharged from hospital. If your medication needs to be kept in a fridge, we will provide this for you and ensure that this is at the correct temperature before your medication is delivered.
Having been safely discharged from hospital and in the comfort of your own home, you may have already been trained to administer your own medication, or alternatively you may require assistance. Our nurses are available to administer your medication or train you to do this yourself.
