Working in partnership with NHS clinicians, we plan the discharge of each patient from hospital and create a tailored care package to meet their specific clinical and social needs.

We can provide all aspects required of a complex and continuing care package, including:

  • case management by appropriately skilled professionals
  • physical and psychological support from the care team
  • equipment, ancillary and prescription management
  • social and environmental support
