HOW TO CLAIM

A step by step guide to making a claim

number 1  FILL OUT YOUR CLAIM FORM

If your dental treatment is covered you will need to fill out the claim form below.

When you visit a dental professional remember to take your claim form with you. Ask the dental professional to complete the Dental Treatment Section of your claim form.

number 1  PAY AND REMEMBER TO GET A DATED RECEIPT

Once you have had your treatment and paid make sure to request a dated receipt so that we can reimburse you.

number 1  RETURN YOUR CLAIM FORM TO US

If you would like to receive payments for claims settlements direct to your chosen bank or building society account instead of posting a cheque to you we can automatically pay your claim by BACS (Bankers Automated Clearing Services).

Check and sign your completed claim form with your dated receipt attached (plus your BACS form if this is your chosen method of payment) and send it back to us at Bupa Dental Cover, Anchorage Quay, Salford Quays, M50 3XL.

Once we have made our payment, we will send you a summary of your claim and treatment details. Please note that payment may take up to two weeks depending on how quickly invoices are submitted to us.

 

 

Are there any waiting periods for treatment?

After you buy, there is a 6 month waiting period before you can claim for oral cancer care, a 4 month waiting period for a dental treatment claim and a 14 day waiting period for emergency dental treatment claims. Please note that the dental treatment waiting period does not apply to routine dental treatment. These waiting periods begin from the policy start date and do not apply at renewal.
