An important change to the SME medical declaration form

We want to make things easier for our customers whenever we can. That’s why we’ve simplified the Group Medical Declaration Form and removed the need to complete it altogether for larger groups.

What’s changed?

Currently, all SME groups with under 100 employees need to complete this form if they are switching to Bupa from another provider. All groups have to answer the same medical questions regardless of size.

From now on only groups with less than 50 employees have to complete the form and the size of the group will determine which questions are asked.

For groups of under 20 employees there is no change – they will answer the same questions as before.

Groups of 20 to 49 employees will answer a shorter version of the questions.

Groups of 50 or more employees no longer have to complete the form.

What are the medical declaration questions?

Here’s a reminder of the existing questions.

These won’t change for groups with less than 20 employees:

Do you know if anyone to be covered on the policy has seen a GP or other healthcare professional, or received treatment (including ongoing medication) in the last two years for any of the following:

  • Heart and stroke conditions including uncontrolled hypertension, angina and heart attacks
  • Any form of cancer
  • Back or spinal conditions, arthritic conditions and or joint replacements
  • Any mental health conditions including stress and depression

Here are the new shorter questions for groups with 20-49 employees:

Do you know if anyone to be covered on the policy has seen a GP or other healthcare professional, or received treatment (including ongoing medication) for any of the following:

  • Any form of cancer in the last two years
  • Back or spinal conditions, arthritic conditions and or joint replacements in the last 12 months
  • Any mental health conditions including stress and depression in the last 12 months

Why have we made this change?

We have listened to feedback and this change removes some of the administration for larger groups.
