Letting us know about a death

Please use this form to let us know that someone has died who has a Bupa policy.

You can upload documents such as a death certificate, executor consent form a coroner’s certificate and next of kin consent form at step four of the form. Don’t worry if you don’t have them yet, please complete and submit it in the meantime.
