Terms and conditions

1. Available to all UK residents aged 18+.
2. Enhance or Peak Health Assessments are subject to availability
3. A Free Bupa SmartDNA kit is only available to customers who purchase either an Enhance or Peak Bupa health assessment at full price either via telephone or online until 31 December 2019.
4. Appointments must be attended in December and the SmartDNA kit will be sent out within 10 working days of Health assessment attendance.
5. The offer excludes all company paid schemes.
6. Bupa reserves the right to cancel, suspend, amend or withdraw the promotion at any point without prior notice where it becomes necessary to do so.
7. Bupa reserves the right to decline to offer an applicant an Assessment in its absolute discretion and Bupa's decision regarding any aspect of this promotion is final.
8. The offer is non-transferable, has no cash value/alternative and cannot be used in conjunction with any other offers or discounts.
9. We recognise that when you give us personal information (which includes health information) you’re trusting us to take good care of it. 10. These terms and conditions shall be governed by English law, and the parties submit to the jurisdiction of the English courts.
